Hello,
We've had to call 911 a few times for my Mom. I'm by no means an expert, but here's a list of things we needed for my Mom's emergencies.
A list of Medications my Mom was on. Health History Dates/ When she had other problems and surgeries Her Insurance Cards A list of her Doctors, with their phone numbers and the type of care they provide for my mom. Her birth date / They may ask at the Hospital for her S.S. # ( I wouldn't write this on the emergency paper to give though)
I've had to make this list up and just copy it several times, one to keep, and one to give to EMTs. I keep one in my Mom's purse, in case we happen to have an emergency while we are out.
My Mom just had several Strokes. And she also has had one Kidney removed, and a heart attack.
When an emergency hits, it's hard to think and remember all of these things.

Excellent post, very important.
You might also add a list of emergency contacts, since you may not always be with your Loved one when something happens.
Additionally, in the case of unmarried persons, especially elderly with no surviving spouse it is important to establish a "Medical Power of Attorney".
