I actually had no problem believe it or not. I called my previous dentist told them I wanted my records because I had health problems and within 1 week I had all the MSDS (Material Safety Data Sheet) including the mercury one.
Now all I have to do is find someone who is willing to take it out without going through a bunch of hoops. This is a another discussion
